>> INSTANT MEDICAL HISTORY

What is Instant Medical History? Established patients and new patients are invited to communicate with our healthcare providers through the new San Juan Healthcare Support Portal. This state-of-the-art software facilitates better communication with your health care provider by providing convenient 24/7 access from the comfort and privacy of your own home or office. Upon entering the Support Portal, you will be asked to respond to multiple choice questions that branch based on your responses. In the background, the software translates the interview summary into clinical terminology and organizes the information before passing the details to and electronic medical record. The Electronic Medical History begins gathering a complete present illness and past medical history before your office visit. Based on your input, the patient interview software organizes your concerns and enables your health care provider to manage your office visit optimally. The output provides your provider with enough information to begin a focused patient interview, saving valuable physician time.

 

>> PRESCRIPTION REFILLS
dr. gossom & patientSpeak with your provider about refilling your ongoing prescriptions at your appointment time.  If you should run out of an ongoing prescription between appointments, please call your pharmacy and they will fax a refill request to our office.  We ask that all patients plan ahead so you have a few pills left prior to calling for a refill.  We require 72 hours (3 days) notice to process most refills.  Most patient chronic medications will need to be checked by the provider on a periodic basis so you may be asked to come in and see your provider before a refill is given.
>> TRAVEL CONSULTATIONS
dr. millerPatients traveling overseas may need specific vaccinations and be alerted to specific medical hazards. Travel consultations involve review of the route and season of travel, the nations being visited, and their intended activities. You will be required to bring detailed information about where you are traveling and how long you will be in each country. Providing the necessary information to your provider will help assess the necessary immunizations required. Sometimes the patient needs to have their medical conditions reviewed in light of the mode of travel (high altitude is one factor).

Some vaccinations require a series of doses over weeks or months. Schedule well before traveling!

You will be scheduled for a ½ hour visit. Please keep in mind that insurance plans do not allow billing of any other medical services at the time of a travel consultation. Medical visits must be scheduled separately. This does not apply to those who pay privately.

Medicare patients will be asked to sign an ABN form and we will process the visit charges through Medicare. Medicare patients should be aware that these claims are usually denied and they may be required to pay those charges at a later date. For patients with all other insurance coverage they should be aware that most insurance plans do not cover travel consultations and you will be required to pay at the time of service.
>> CONTINIOUS GLUCOSE MONITORING
San Juan Healthcare is now offering a new service for patients with Diabetes (type 1 and 2). In pursuit of our continuing goal to provide the best care possible and utilizing the most current technology, we are offering a new type of glucose measurement – continuous glucose monitoring. This is a device that uses a sensor, a small needle inserted into the subcutaneous tissue, which obtains a blood sugar every 3 minutes for 72 hours. The readings are sent to a small device worn on the skin. This information is then downloaded and can be analyzed to assist our diabetic patients in management of this disease. If you are interested in this new service, you can make an appointment with your provider.
